All three are surgical sperm retrieval techniques used when a man has azoospermia (no sperm in ejaculate). PESA (Percutaneous Epididymal Sperm Aspiration) involves a needle aspiration from the epididymis and is used for obstructive azoospermia — where sperm are produced but blocked from reaching the ejaculate. TESA (Testicular Sperm Aspiration) takes a small tissue sample from the testis and works for mild non-obstructive cases. Micro-TESE is the most advanced technique, using an operating microscope to identify and extract rare sperm-producing tubules in the testis — it is recommended for non-obstructive azoospermia where sperm production is severely impaired. A hormone panel (FSH, LH, testosterone) and testicular ultrasound help predict which technique will be most successful.
